The survey form was developed collaboratively between SOAM’s staff and board members and Health-Tech Consultants, Inc., the contracted external evaluation team. The survey was distributed under one link with two parts. A convenience sampling method was used to collect responses. Respondents were asked to answer 12 core questions, then invited to answer 23 questions on a voluntary basis. This summary provides predominant results of that survey.
